Seven AI and live receptionists ranked for dental front desks: BAAs, practice management booking, new patient and recall calls, and after hours emergencies.
By , research by the BestBots team ยท Updated Oct 19, 2026
Fact checked against each company's published pricing, documentation and verified user ratings. How we score
Weave is the best AI receptionist for most dental offices: it is the only option we compared that books directly into Dentrix, Eaglesoft, Open Dental and Planet DDS, and it is built for HIPAA. Arini is the best alternative if you want a dental only specialist that also runs recall and reactivation calls.
Our top picks
We compared seven AI and live receptionists on what their pricing pages, product pages and review listings say as of October 2026. Scores follow our published rubric.
How to choose
A dental front desk loses calls at the worst times: at 8 a.m. when everyone calls to confirm, at lunch, and while the team checks out a patient. Judge each tool on these points:
- BAA first. Callers give names, dates of birth, insurance and symptoms. That is protected health information, so the vendor must sign a BAA.
- PMS booking. "Books appointments" can mean writing into Dentrix or texting a Calendly link. Ask which, on the plan you will buy.
- New patient vs existing patient. New patients bring the most production. Check that the tool captures insurance carrier, reason for visit and preferred times.
- Recall and reactivation. Outbound calls to overdue patients fill hygiene chairs. Arini and Hello Patient list outbound work; most general tools only answer.
- After hours triage. Swelling and trauma need a different path from a lost crown.
- Languages. Arini and Upfirst detect and switch languages; Hello Patient covers English and Spanish.

HIPAA and BAAs
Under HIPAA, any vendor that handles patient information for your practice is a business associate, and the contract has to cover how it uses and protects that data (45 CFR 164.504(e)). For an AI receptionist, that means call recordings, transcripts, texts and anything written into your schedule.
What each record says:
| Tool | HIPAA or BAA, as listed |
|---|---|
| Weave | Built for HIPAA, per Weave |
| Arini | Not stated; ask for a BAA |
| Hello Patient | SOC 2 Type 2, signs BAAs, no training on patient data |
| Nextiva XBert | Lists HIPAA compliant |
| Upfirst | BAA on custom plans above 600 calls |
| AnswerConnect | HIPAA option in its portal |
| Ruby | HIPAA compliant service as an add on |
Get the signed BAA before the first patient call, not after. Then ask two more questions in writing: does the vendor use your call data to train its models, and how long does it keep recordings. Hello Patient is the only record that answers the training question on its own pages.
Practice management booking
Weave is the only record that names dental systems. Through TrueLark by Weave it books into Dentrix, Eaglesoft, Open Dental and Planet DDS, and only for qualified PMS setups, so confirm your version qualifies. Arini says it works with cloud and on premise PMS and shows logos without naming them. Get the list in writing.
Everyone else books into a general calendar. Nextiva uses Google Calendar, Calendly, Cal.com and Zoho; Upfirst uses Google Calendar, Outlook, Calendly and Acuity. That works for a request queue your front desk confirms each morning, but it is not a live schedule. If a hygienist's column shows open in Dentrix and booked in Calendly, you will double book.
New patient, insurance and recall calls
New patient calls are worth the most and take the longest. Script the AI to capture full name, date of birth, phone, insurance carrier and subscriber, reason for visit and preferred days. Tell it which carriers you take as in network and what to say about the rest. It should not quote a patient's out of pocket cost; that depends on their plan.
Existing patients mostly call to confirm, reschedule or ask about a bill. Confirmations are where AI saves the most staff time. Arini lists confirmations and Weave pairs the AI with texting, so a patient can confirm by reply.
Recall is outbound. Arini lists reactivation campaigns and Hello Patient lists recall outreach. A practice with 500 overdue patients can fill hygiene openings without tying up the desk, but check your state's rules on automated calls and get consent for texts.
Dental emergencies after hours
Write a short triage script with your dentist and give it to the vendor. Facial swelling, trouble swallowing or breathing, trauma to the face or uncontrolled bleeding go to your on call dentist, or to 911 or the emergency room for breathing problems. A knocked out tooth goes straight to the on call dentist. Pain without those signs, a lost filling or a broken crown gets the first emergency slot the next morning.
The AI does not diagnose. Its job is to ask the same questions every time, route the urgent calls and book the rest. AI tools transfer to your on call phone; AnswerConnect and Ruby put a live person on the line overnight if your dentist prefers that.
What it costs
Two volumes: one office taking 600 calls a month at 3 minutes (1,800 minutes), and a three office group taking 1,800 calls (5,400 minutes). Each cell is the cheapest published plan on monthly billing that covers the volume, with overage at the listed rate.
| Tool | 1 office: 600 calls | 3 offices: 1,800 calls |
|---|---|---|
| Upfirst | $299 (no BAA on Scale) | Custom quote (BAA plans) |
| Nextiva XBert | $594 | $1,782 |
| Weave | Quote only (plans from $199) | Quote only |
| Arini | Quote only | Quote only |
| Hello Patient | Quote only | Quote only |
| AnswerConnect | Quote only | Quote only |
| Ruby (live only) | Quote (above 500 minute plan) | Quote |
The math:
- Upfirst: Scale covers 600 calls for $299. At 1,800 calls the published rate would be $299 plus 1,200 at $0.70, or $1,139, but you need a custom plan above 600 calls to get a BAA, so ask for that quote.
- Nextiva: $99 for 100 interactions plus 500 at $0.99 for one office; plus 1,700 at $0.99 for the group.
- Ruby: its largest published plan is 500 minutes for $1,725, a third of one office's volume; overage is not published.
Most of this category is quote only, so get two written quotes at your real call count. A full time front desk hire at the BLS median of $18.27 an hour costs about $3,167 a month in wages alone, and many offices staff two. Run your volume in the cost calculator, track rates in the AI Price Index, and see the full breakdown in AI receptionist cost.
Red flags
- No signed BAA. Do not route patient calls to a vendor that will not sign one.
- "Integrates with your PMS" without names. Ask for the exact system and version, in writing.
- Calendar booking that does not touch the PMS. It causes double booked chairs.
- AI that quotes coverage. Insurance estimates come from your team.
- No emergency script. Swelling and trauma must reach a dentist.
- Quote only with a long contract. Ask for the term, cancellation rules and setup fees before the demo ends.
How we picked
We used published facts only: pricing pages, product pages, BAA pages and review listings, as of October 2026. We did not place test calls, so we do not grade voice quality. Factor weights are value 25%, features 25%, trust 20%, ease 15% and integrations 15%, as described on how we score. Order is editorial, set on fit for dental offices: dental PMS booking, BAAs, published pricing at a dental office's call volume and after hours handling.
Smith.ai, Rosie, Goodcall and Dialzara were left out because their records list no BAA or HIPAA terms and no dental software. Medical practices should see AI receptionists for medical offices.
Frequently asked questions
What is the best AI receptionist for a dental office?
Weave is the best fit for most dental offices because it is the only option here that names Dentrix, Eaglesoft, Open Dental and Planet DDS for direct booking, and it is built for HIPAA. Arini is the best dental only alternative if you also want reactivation calls, but ask for its PMS list and BAA in writing.
Is an AI receptionist HIPAA compliant?
Only if the vendor signs a business associate agreement with your practice and protects patient data under it. Hello Patient signs BAAs, Upfirst offers them on custom plans above 600 calls, Ruby sells HIPAA as an add on, AnswerConnect has a HIPAA option, and Nextiva lists HIPAA compliance. If a vendor will not sign a BAA, do not let it handle patient calls.
Can an AI receptionist book into Dentrix or Open Dental?
Weave books directly into qualified dental PMS, including Dentrix, Eaglesoft, Open Dental and Planet DDS. Arini says it syncs with cloud and on premise PMS but does not name them. The general purpose tools book into Google Calendar or Calendly, which your team then copies into the schedule.
How much does an AI receptionist for a dental office cost?
On published plans, a single office taking 600 calls a month pays $299 on Upfirst Scale or $594 on Nextiva XBert. Weave's AI tier, Arini, Hello Patient and AnswerConnect are quote only, so request a written quote at your call volume.
Can AI handle dental insurance questions?
It can answer the questions you script, such as which plans you accept as in network and what to bring to the first visit. It should not quote coverage or out of pocket cost for a specific patient; that needs your team and the patient's plan details. Hello Patient lists insurance questions among its intake tasks.
What should an AI receptionist do with a dental emergency after hours?
Follow your triage script: collect the patient's name, number and complaint, and send swelling, trauma or uncontrolled bleeding to your on call dentist or to emergency care. The AI does not diagnose. Routine pain without those signs can be booked into the first emergency slot.
